~ Entering Waiver Data into eFORCE and Distributing Paperwork
Introduction
While the counselor or teacher may enter Waiver data into eFORCE, support staff are responsible for ensuring the Waiver information and case note are entered into eFORCE. The IL Administrative Assistant files the signed Waiver form into the paper case file.
Entering Waiver data requires:
1. Entering the Waiver Case Note.
Entering the Waiver Case Note
The following details the steps to enter the Waiver case note into eFORCE.
Step 1
Open the client’s record in eFORCE.
Step 2
Open a blank case note page by choosing the CaseNotes link in the Navigation bar. Then, choose the New Case Note button on the Case Notes page.
Step 3
Complete all necessary fields for this case note.
Guidelines for the Waiver Case Note fields:Enter the plan date into the Date field.
In the Note Type field, choose the General option.
In the Subject line, enter “New Waiver.” (If additional information is provided in the case note, reference that in the subject line as well.)
Step 4
Choose the Spell Check button.
The text from the case note fields will be copied into Microsoft Word.
If spelling errors are found, the Spelling dialog box will prompt you to choose a correction or ignore the word.
If no errors are found, a dialog box will indicate that “Spellchecking has been completed.” Choose the OK button to return to the Plan page.
Step 5
Choose the Submit button.
If you have not entered information into required fields, a dialog box will indicate which fields need data. (A red asterisk will appear next to the field on the page.) Complete the fields as required. Then, choose the Submit button again.
Completing the Waiver Plan in eFORCE
The following details the steps to complete the Waiver Plan page in eFORCE.
Step 1
With the client’s record open in eFORCE, choose the IL Case Info link in the navigation bar.
The Case Summary page will open.
Step 2
Choose the New Plan button under the Plan History heading.
The Plan page opens.
So far, the Plan page lists the Case Type, Case ID, and Status.
Notice this note at the top of this page: Warning: Creating a New Plan will render all other plans obsolete.
Step 3
Choose Waiver in Plan Type field. Then, enter plan date into the Plan Date field. (This is a required field.)
Step 4
Choose the Create New Plan button.
A blank Waiver Plan page opens. (A note appears at the top of the page indicating a new plan has been created. All other plans are now obsolete.)
Step 5
Under the Independent Living Goal heading, complete the Goal Description and To Be Attained By fields. (This information should match what is written on the waiver.)
Step 6
Add objectives by choosing the Add Objective button.
The eFORCE New Objective window will open.
Step 7
Complete the Objective Type, Objective Set By, Objective Status, Objective Description, and Date Set fields for each objective to be listed on the Waiver plan.
Guidelines for adding objectives:
The Date Set field is a required field.
Enter the objective exactly as it is written on the plan. The wording on a signed Waiver must match the wording in eFORCE.
Step 8
Choose the Spell Check button.
The text from the Waiver plan fields will be copied into Microsoft Word.

If no errors are found, a dialog box will indicate that “Spellchecking has been completed.” Choose the OK button to return to the Plan page.
Step 9
Choose the Submit button.

If you have entered information correctly, a note will appear at the top of the page indicating that the plan information has been saved.
Distributing Paperwork
Once the Waiver plan has been entered, the IL administrative assistant files the handwritten, signed Waiver form into the client’s paper case file under the Plans and Amendments tab.
